Testicular Sperm Extraction (TESE) is a surgical procedure performed by a urologist. A sample taken from the testicle is transferred to the laboratory for subsequent examination, where the embryologist analyzes how viable it is.
The procedure takes place under general anesthesia. The testicle is incised to take a sample of the material. This is a quick procedure in which the patient does not need hospitalization. The analysis of the material is carried out by an embryologist with the use of a microscope and, if necessary, additional treatment is prescribed. A high sperm rate is observed during the TESE procedure, with the exception of some cases where large fragmentation and Sertoli cells occur only in Y chromosomes where it is not seriously affected by sperm production.
Testicular Sperm Aspiration (TESA)
This procedure is usually performed under general anesthesia and involves the extraction of testicular tissue from the testes. The biopsy is collected into a sterile tube containing the nutrient culture fluid, avoiding, if possible, blood from entering there, and transferred to the andrology laboratory for further examinations. Using very fine needles, the embryologist divides the tissues into individual tubules, potentially containing sperm. With gentle agitation or shaking, the spermatozoa are separated in such a way that they can be concentrated by careful centrifugation. TESA procedure is relatively quick.
The patient is recommended to use sterile wipes and antiseptic solutions to clean the wound, while courses of antibiotics and painkillers are prescribed to eliminate possible infection and/or local inflammation.
The only difference between TESA and TESE procedures is that when micro TESE samples are examined under a microscope. In this case, both chances of finding a sperm sample will increase, and the damage to the remaining testicular sample will be minimized.
